Hyperkalemia and atrial pacemakers: what can the electrocardiogram tell you?

چکیده
A 73-year-old white male with a history of coronary artery disease, congestive heart failure, diabetes mellitus, and pacemaker/implantable cardioverter defibrillator placement presented to the emergency department with a chief complaint of abnormal laboratory tests. On arrival, he complained of nausea, vomiting, diaphoresis, and shortness of breath. Physical examination revealed pulmonary rales and peripheral edema. His presenting and previous ECGs are shown in Figure 1 and Figure 2, respectively. The patient was found to be hyperkalemic secondary to acute renal insufficiency, and after standard emergency department treatment and emergent dialysis, the patient was stabilized and discharged home in 2 days.

Spontaneous Sinus Conversion of Permanent Atrial Fibrillation During Treatment of Hyperkalemia
Hyperkalemia is a common adverse effect of treatment for heart failure and is associated with high mortality and morbidity. The cardiac manifestations of hyperkalemia include various electrocardiogram changes. We describe a case of a 74-year-old woman with heart failure and permanent atrial fibrillation who reverted to normal sinus rhythm during recovery from hyperkalemia.
متن کاملElectrocardiographic diagnosis of hyperkalemia in the presence of ventricular pacing and atrial fibrillation.
H yperkalemia has several electrophysiologic effects on cardiac tissues that allow for its ready detection on a standard electrocardiogram. The diagnosis is easiest when the baseline tracing is normal with sinus rhythm and a normal QRS duration. We report this case of hyperkalemia which was diagnosed in the presence of ventricular pacing and atrial fibrillation and which resulted in the convers...
متن کاملArrhythmias and ECG changes in life threatening hyperkalemia in older patients treated by potassium sparing drugs.
BACKGROUND Severe hyperkalemia is a life threatening condition that can cause fatal rhythm disturbance and terminal heart arrest. The most common cause of hyperkalemia in older patients is that of iatrogenic medication-related etiology due to associated polymorbidity, polypharmacy and reduced reserve metabolic capacity. The aim of this paper is to increase awareness in the clinicians of the ris...
متن کاملSliding Mode Control For Heartbeat Electrocardiogram Tracking Problem
In this paper, we have exploited the first-order sliding mode control method to track the ECG data of the human heart by three different nonlinear control laws. In order to lessen the intrinsic chattering of the classic sliding mode control system, smooth function approximations of the control input, by means of the hyperbolic tangent and the saturation function, were used. The fast Fourier tra...
متن کاملHyperkalemia-induced complete heart block
Background: Potassium, as an extracellular ion, plays an important role in the electrophysiologic function of the myocardium and any change in extracellular concentration of this ion might have a marked impression upon myocyte electrophysiologic gain. High serum potassium levels are thought to impair pulse conduction in Purkinje fibers and ventricles more than that in the Atrio...
